Taiwan's healthcare system faces sustainability test amid ageing population


Taiwan's National Health Insurance (NHI) system remains one of the most accessible healthcare systems in the world. However, Nan Shan Life Insurance Chairman Chung Yao Yin said in an exclusive interview with Asia Insurance Review that mounting demographic and financial challenges are dimming the market's long-term outlook.

Mr Chung said Taiwan has near-universal coverage, citing its affordability and convenient medical network. These strengths, according to the official, have helped build strong public trust and social stability over the years. However, he warned that accessibility and sustainability are beginning to diverge as healthcare costs continue to rise.

“As advanced treatments, such as precision oncology and robotic surgery, continue to evolve, the gap between what people expect and what the system can realistically fund is gradually widening,” hesaid.

He added that Taiwan is now at a critical crossroads, and decisions made over the next decade will determine the fate of the NHI and whether it will remain a long-term national strength.

Mr Chung noted that Taiwan has entered “super-aged society” status, with more than 20% of the population now aged 65 and above. This, he said, will significantly increase demand for chronic disease management and long-term care services. At the same time, NHI premiums are largely funded by the working population, whose numbers are gradually shrinking.

“Pressure is building from both sides simultaneously, rising utilisation and a narrowing funding base,” he said.

He also pointed to growing public expectations that the NHI should provide access to nearly any treatment at minimal cost. While understandable given the system’s history, he said this creates political challenges for reform. “Deferring it further will only narrow our options,” he said.
